Insurance & Sliding Fee

At Family First Health, we accept most insurances and our fees range depending on the types of visit and tests that are performed. Using a sliding fee scale, we can provide discounts based on family size and household income. Our registration staff will be glad to explain our sliding fee scale system to you when you come in for your visit. We will not deny medical services to anyone due to an inability to pay.

At Family First Health, fees range depending on the types of visit and tests that are performed. To apply for our Sliding Fee Scale Discount Program, you may print and complete the sliding fee scale discount program application form by clicking one of the following links. Health Insurance Marketplace
The Marketplace is a way to find health coverage that fits your budget and meets your needs. With one application, you can see all your options and enroll. You may even qualify for lower costs for monthly premiums and out-of-pocket costs on private insurance based on your household size and income. Some people with very low incomes may wind up paying very small premium – or no premiums at all.

To get an idea of what you may qualify for, check out the subsidy calculator. You can enter different income levels, ages, and family sizes to get an estimate of your eligibility for subsidies and how much you could spend on health insurance. Find more information here.

Medicaid
Medicaid provides coverage to millions of Americans with limited incomes or disabilities. Each state’s Medicaid rules are different. You can apply for Medicaid now (link to gov site) to see if you’re eligible. Or you can fill out a Health Insurance Marketplace application when open enrollment begins. Find more information here.

Medicare
Medicare is the federal health insurance program for people who are 65 or older, certain younger people with disabilities, and people with End-Stage Renal Disease (permanent kidney failure requiring dialysis or a transplant, sometimes called ESRD.) Find more information here.

Children’s Health Insurance Program (CHIP)
CHIP provides coverage for children, and in some states pregnant women, in families with incomes too high for Medicaid but too low to afford private insurance. You can apply for CHIP now to find out if you’re eligible, or you can fill out a Marketplace application when open enrollment begins. Find more information here.

Payment Plans
We know that everyone faces financial burdens from time to time, and sometimes it isn’t always possible to pay your bills on time. Clients may wish to make payment plans for medical bills to pay any fees for services rendered at Family First Health. We will always be willing to work with you to make appropriate payment plans for medical bills that work with your budget and health needs. We will never deny health care to anyone because of an inability to pay. If you would like to arrange a payment plan, please contact our Billing Department.

Deductibles & Co-Pays
While all Family First Health centers accept most forms of insurance, some insurance plans require the patient to pay deductibles and copays before they will pay for services with us. These deductibles vary among insurance companies. Even within an insurance company, different insurance plans may have different deductibles. If your insurance plan has a deductible, you must pay that amount before the insurance company will make payments to us. You are responsible for paying this deductible.

In addition, some insurance companies may require you to pay a co-pay for an office visit. A co-pay is a small amount that you must pay at each visit to your medical provider. The amount of the co-pay varies depending upon your insurance company and your plan. If your insurance plan has a co-pay, we will charge you that amount at the time of service, and you are responsible for this fee.

To find out if your insurance plan has a deductible or a co-pay, you should contact the insurance company’s customer service department (the phone number can usually be found on your insurance card.) If your insurance plan is offered through your employer, you can also find out about any deductibles or co-pays from your employer.

It is very important that your co-pay is paid in full the day of your visit so we can continue to provide quality healthcare to you and the community.
